Office Policies & Procedures

Our Legal Duty

We are available to schedule appointments Monday through Friday from 8:30am – 4:00pm. We kindly ask that you give at least 24 hours notice if you are unable to keep your scheduled appointment with us.

Prescription Refills

Prescription refills should be handled during business hours Monday through Friday. We require 48 hours notice when you need your prescription refilled. Be prepared to provide us with your pharmacy phone number when you call.

No narcotics refills will be called in on nights or weekends.

Privacy Policy

We take the privacy of your personal health information seriously. You will be provided with our privacy policy upon registration and it is also available online by clicking here. We comply with all HIPAA guidelines.

Accepted Insurance

We accept most health insurance plans. Please click here for a complete listing.

Payment for Services

You will be asked to pay for any portion of your bill that is not covered by your insurance such as co-payments, deductibles and non-covered services. We accept cash, checks and major credit cards.
